The public service broadcaster committed a "significant breach" of broadcasting rules by not revealing that the presenter of a programme about Gaza was the offspring of a Hamas administrator, as ruled by regulatory body Ofcom.
An inquiry by Ofcom into Gaza: How to Survive a Warzone has determined that the programme was "significantly deceptive".
The chief executive of the broadcaster had previously apologised, noting there had been "a major oversight in relation to accuracy".
The authority has directed the corporation to air a peak-time announcement about its findings.
"Since this amounts to a serious breach of our code, we are directing the BBC to transmit a announcement of our determinations against it on BBC Two at 21:00, with a schedule to be confirmed," it said.
'Potential to Damage Confidence'
The authority's announcement stated: "The inquiry we conducted determined that the programme's lack of disclosure that the voice's parent served in a role in the Hamas-controlled authority was materially misleading.
"It meant that the audience did not have critical information which may have been extremely pertinent to their evaluation of the presenter and the details he provided.
"Trust is at the core of the relationship between a media organization and its audience, particularly for a public service broadcaster such as the BBC.
"This error had the possibility to undermine the significantly high amounts of confidence that viewers would have put in a factual film from the broadcaster about the Israel-Gaza war."
The documentary was pulled from the online platform in last February after the presenter's personal associations emerged, and in this past July an broadcaster's internal assessment concluded it breached the corporation's editorial guidelines on truthfulness.
A corporation official commented: "The Ofcom ruling is in agreement with the findings of Peter Johnston's review, that there was a major oversight in the programme in relation to the BBC's Editorial Guidelines on factual correctness, which corresponds to Rule 2.2 of the authority's broadcasting regulations.
"We expressed regret for this and we comply with the authority's determination in full. We will comply with the sanction as soon as the timing and language are determined."
